Name	Required Optional Conditional	Edits/Business Rules
Applicant/Application Information	R	Read-only for this screen.
Supplement Application	R	Supplement application number. The Application link provides a detailed view of application information.
Supplement Cycle	R	Cycle number defaults to “1” when an application is created; updated if more than 1 cycle is required. Authorized users can select any valid cycle number for the selected application. This provides a view of application information as it appeared for the selected cycle.

Date Attached	R	Date the document was attached. Populated by the system when the attachment is made.
Document Name	R	File name of the document selected from the Browse window.
Document Size (MB)	R	Physical file size measured in bytes. Populated by the system when the attachment is made.

PATA Documents Section

Name	Required Optional Conditional	Edits/Business Rules
Document Name	R	File name of the document selected from the Browse window. PATA documents are listed numerically; new documents are incorporated into the list in numeric order.
Document Description	R	Description of document name that is selected form the Browse window.
Attached By	R	Name of the user who attached the document. Populated by the system when the attachment is made.
Date Attached	R	Date the document was attached. Populated by the system when the attachment is made.
