First line of customer address. Required to activate Utility account.

Insurance Coverage Section

 

Displays insurance information for the customer account being processed.

The following business rule applies: a customer account has only one associated entry in the CIMS Insurance table.

Insurance Type

Indicates if this Customer has a Self-Insured Agreement, commercial insurance, or both. Drop-Down. Selection includes:

The following business rule applies: a customer account only has one associated entry in the CIMS Insurance table.

Note: When a customer account is activated, it must have in-force insurance coverage, i.e. Confirmation Date is greater than or equal to the current date.

Insurance Confirmation Date

R

Date the insurance coverage for the Customer being processed needs to be re-confirmed by the Account Manager.

Must be greater than or equal to the current date.

Note: Current procedures are for Account Managers to confirm Self-Insured coverage by April 30 of each year. When the type of coverage is Self-Insured and Commercial Insurance (both), the Account Manager will enter the earliest date the coverage needs to be re-confirmed. These notes are related to PennDOT procedures not CIMS business rules.

Security Information Tab

Security Type

The security type can be selected from a dropdown list. Selections are Bond Security Number or Letter of Credit Security Number. Field names are influenced by the security type selected.

Bond/Letter of Credit Number

O

An auto-populated Bond Security Number/Letter of Credit Security Number (8 digit numerical field) formatted as the last two digits of the current year, followed by two digits for the month with a zero preceding if it's a single digit, two digits for the date of the month with a zero preceding if it's a single digit and lastly an extension number which is determined by the number of bonds/letters of credit processed on that date, when a bond or letter of credit value is indicated.
